For your first visit with Biehl Dentistry, we ask you to fill out our New Patient Form
that provides us with some basic information describing your current condition, a brief health history and general information.

Do you accept my insurance?
As a service to our patients, our practice is a member of Delta Dental Premier Network. We are also in network with Ameritas, Aetna, Guardian and United Concordia. Our accounting staff will file insurance for you. However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for knowing benefits and providers that you are able to see.
What forms of payment do you accept?
For your convenience, we accept cash, personal checks and major credit cards (Visa, Mastercard, American Express, Discover). Payment is expected at the time services are performed. When more extensive dental care is necessary, financing arrangements can be made with:
